Do I need to present or show an ID (identification card) to enroll?
It is not necessary to present an ID upon enrolling as a member client, but, in the end your MSA name must match your data on your death certificate, or else you won't be covered.
How to Apply for a Membership Package?
• Contact your trusted Insurance agent affiliated with California Garden.
• Ask for a MEMBER/CLIENT form
• Choose your membership package
• Fill up the membership form
• Pay the equivalent amount of your chosen MSA
• Add a 1 time pay of P50.00 as processing fee
• Ask for receipt

What is MSA?
MSA mean MEMORIAL SERVICE ARRANGEMENT
this includes:
• Pick-up of remains from residence or hospital
• Embalming for 3 or 5 days viewing (includes dressing and make-up of the remains)
• Casket assigned in your MSA
• Use of a chapel (if included in your MSA)
• Expanded assistance to include free use of lighting equipment, funeral hearse for the interment.
• Assistance in documentation
• Depending on your MSA
this includes:
